چکیده

Neurosurgery requires the use of osmotic agents to reduce intracranial pressure which can adversely affect coagulation. Thromboelastography provides early indicator of derangement of coagulation. A prospective, non randomized pilot study was conducted to compare the effect of administration of either mannitol or hypertonic saline and when combined with hydroxy ethyl starch on coagulation and platelet function, as measured by Thromboelastography. 22 neurosurgical patients, divided into 2 groups, received 20% mannitol (n=12) or 3% hypertonic saline (n = 10) followed by administration of hydroxy ethyl starch (HES). The Thromboelastography parameters were comparable in both the groups at baseline; after administration of the drug and also after HES administration. The results show that combining a single dose of either mannitol or hypertonic saline with hydroxyl ethyl starch does not appear to affect in-vivo coagulation.

Comparison of the effects of 7.2% hypertonic saline and 20% mannitol on whole blood coagulation and platelet function in dogs with suspected intracranial hypertension - a pilot study

BACKGROUND Hyperosmolar therapy with either mannitol or hypertonic saline (HTS) is commonly used in the treatment of intracranial hypertension (ICH). In vitro data indicate that both mannitol and HTS affect coagulation and platelet function in dogs. The aim of this study was to compare the effects of 20% mannitol and 7.2% HTS on whole blood coagulation using rotational thromboelastometry (ROTEM...

In vitro effects of 3 % hypertonic saline and 20 % mannitol on canine whole blood coagulation and platelet function

BACKGROUND Hyperosmolar therapy, using either mannitol or hypertonic saline (HTS), is considered the treatment of choice for intracranial hypertension. However, hyperosmolar agents may impair coagulation and platelet function, limiting their use in patients at risk for hemorrhage. Despite this, studies evaluating the effects of mannitol compared to other hyperosmolar agents in dogs are largely ...

Agreement of measured and calculated serum osmolality during the infusion of mannitol or hypertonic saline in patients after craniotomy: a prospective, double-blinded, randomised controlled trial

BACKGROUND Mannitol and hypertonic saline are used to ameliorate brain edema and intracranial hypertension during and after craniotomy. We hypothesized that the agreement of measured and calculated serum osmolality during the infusion of hypertonic saline would be better than mannitol. The objective was to determine the accuracy of serum osmolality estimation by different formulas during the ad...

Comparison of equiosmolar concentrations of hypertonic saline and mannitol for intraoperative lax brain in patients undergoing craniotomy

BACKGROUND Osmotherapy is the frequently used for the treatment of intracranial pressure. The purpose of the study was to compare the effect of equiosmolar solution of 3% hypertonic saline and 20% mannitol on brain relaxation in supratentorial tumor surgery. METHODS After institutional review board approval and written informed consent, 50 patients aged >18, Glasgow Coma Scale (GCS) >13 with ...

Comparison of 7.2% hypertonic saline - 6% hydroxyethyl starch solution and 6% hydroxyethyl starch solution after the induction of anesthesia in patients undergoing elective neurosurgical procedures

OBJECTIVE The ideal solution for fluid management during neurosurgical procedures remains controversial. The aim of this study was to compare the effects of a 7.2% hypertonic saline - 6% hydroxyethyl starch (HS-HES) solution and a 6% hydroxyethyl starch (HES) solution on clinical, hemodynamic and laboratory variables during elective neurosurgical procedures. METHODS Forty patients scheduled f...
